Grant Reports

Herzstein Foundation grantees must furnish brief written reports at the end of their grant periods. Please use the appropriate form below to create your report and e-mail it to mail@herzsteinfoundation.org. Note that there is a special version if your organization received funds for a capital campaign. If your organization’s funds benefited an endowment, refer to Exhibit A in your grant agreement. Please also feel free to send us links to any articles, YouTube videos, or audio clips demonstrating the work achieved as a result of a gift from the Herzstein Foundation.
